Scouting America requires all members to complete the Annual Health and Medical Record  (AHMR) annually for all activities. Parts A & B are required for all Scouting events, day camps, and trips under 72 hours. Part C must be completed by a medical provider, and is mandatory for resident camps or events exceeding 72 hours. Schedule your physical today! Appointments are filling up fast at many primary care offices, and urgent care will not always fill out this form.
